Learn about
Inborn Errors of
Metabolism (IEM)

We guide families and clinicians through IEM. What it is, how to screen, and where to find expert care.

What are Inborn Errors of Metabolism (IEM) ?

Inborn Errors of Metabolism (IEM) are a group of genetic conditions that affect how the body uses food for energy and growth. Our bodies rely on enzymes to break down nutrients like proteins, fats, and sugars. In IEM, one of these enzymes does not work properly or is missing. As a result, toxic substances can build up, or the body may not be able to produce enough energy or essential molecules.

Inborn Errors of Metabolism (IEM) are genetic conditions where the body cannot properly process certain nutrients such as proteins, fats, or sugars. This can cause toxic substances to build up or prevent the body from producing essential energy.

Many IEMs are rare on their own, but together they affect a significant number of babies and children. The good news is that several IEMs are treatable, especially when they are found early through newborn screening and careful follow‑up.

How do IEMs Happen?

How do IEMs Happen?

IEMs are usually caused by a change (mutation) in a single gene. This gene carries instructions for making a specific enzyme or protein involved in metabolism. When the gene is altered, the enzyme may not work as it should.

Most IEMs are inherited from both parents in an autosomal recessive pattern. This means:

1

Both parents are typically healthy carriers.

2

Each pregnancy has a 25% chance of the child being affected, a 50% chance of being a carrier like the parents, and a 25% chance of being unaffected and not a carrier.

These conditions are present from birth, but symptoms may appear at different ages, from the newborn period to later childhood or even adulthood, depending on the specific disorder.

Signs of IEM

If a child shows these symptoms repeatedly or during illness, metabolic evaluation may be needed. Do not ignore your concerns, speak to a doctor and ask if an Inborn Error of Metabolism could be the cause.

1

Recurrent vomiting

2

Excessive sleepiness or lethargy

3

Seizures

4

Developmental delay

5

Poor feeding

6

Illness after fasting or infection

7

Unusual body odour

01

Newborn Screening

Soon after birth, a few drops of blood are collected from the baby’s heel and analysed for certain treatable conditions, including some IEMs. Abnormal results need quick follow‑up tests to confirm the diagnosis.

Laboratory Tests

Blood and urine tests can measure specific chemicals, acids, or amino acids that point to a particular metabolic pathway.

03

Genetic Testing

DNA tests can identify the exact gene change causing the condition. This helps confirm the diagnosis and guide family planning.

Specialist Assessments

Metabolic specialists, paediatricians, neurologists, and dieticians work together to assess the child’s growth, development, and response to treatment.
